I was indeed using 0.8.53. My apologies for not paying enough attention. I will retry the rewrite module with 0.8.54.

The symptom seems like you're using rewrite_by_lua with nginx 0.8.41 ~
0.8.53. There was a famous regression in that range of nginx versions
as described here:

I've stated in ngx_lua's documentation that rewrite_by_lua won't work
with nginx 0.7.x and 0.8.41 ~ 0.8.53.
