Error Log Client IP Missing

So, I have an issue with error log.
Sometimes, it doesn't show client IP. Example is here:

In first [error] message I can see "client: 1.2.3.4" block, which shows the IP.
But in the second one [error] message it seems to me, that I don't see IP because of the particular reason - PHP Error Message overflows some buffer which Nginx provides for error message, so the Nginx logs entry without any other additional information.
Is that correct assumption?
If so - can I change the size of the buffer somehow?
And is it possible at all to change error log's format? For instance place "client 1.2.3.4" as first entry in the log line?

I mean, probably everything is possible if you take sources and write your own version with custom tweaking, but I'm looking for existing tools/settings. Thank you!
