Update on NGX_HTTP_SUBREQUEST_WAITED

A little more investigation shows that the reads seemingly are being done in
order; however, all of the connections are opened 'up front' -- and
seemingly are held open until the read is done; For our application, it
would be more desirable to not even open the connection to the back-end
until the read is about to occur. Is the current situation by-design?
