Для начала - смотреть в конфиг и логи. В частности, из
access_log'а убедиться, что запрос от действительно доходит, и
обрабатывается именно там, где вы ожидаете. А из error_log'а -
что ошибок нет.

Just in case, у меня по приведённой вами ссылке всё замечательно
стримится - но при этом используется HTML5 video и range-запросы,
никакого flash'а.
