It's possibly how mod_rpaf is configured in your apache.conf file.

Make certain that mod_rpaf is loaded and use something like this (mostly
from page previously linked):

LoadModule rpaf_module /path/to/modules/mod_rpaf-2.0.so

RPAFenable On
RPAFsethostname On
RPAFproxy_ips 127.0.0.1
RPAFheader X-Forwarded-For
